In-House Patient Savings Plan

Designed for patients with NO dental insurance

Our In-House Patient Savings Plan gives you the opportunity to obtain your dental care at reduced prices! This plan is NOT dental insurance. It cannot be used with any form of insurance, workers’ compensation claims, coverage under automobile, medical, any patient financing (CareCredit); or any other insurance claims associated with our office. The In-House Patient Savings Plan allows you to obtain your dental care at reduced prices!

• NO pre-existing conditions
• NO pre-authorization requests
• NO waiting periods

All other services are discounted at 20% of our usual and customary fee. Services include, but are not limited to:

• Periodontal Therapy/Scaling and Root Planing
• Fillings
• Crowns
• Bridges
• Implants and Implant Restorations
• Cosmetic Dentistry
• Clear Aligner Orthodontics
• Root Canals
• Extractions
• Bone grafting
• Partial/Full Dentures

$375 Adult Plan Membership includes:

• Two regular dental cleanings (deep cleanings/scaling and root planing not included).
• One initial comprehensive exam (includes checking for tooth decay, gum disease, cosmetic problems, bite and jaw problems, and oral cancer)
• One annual exam
• One full set of X-rays every five years, four bitewings once a year (with annual exam) and individual
• X-rays as needed
• One emergency exam during the one-year period (treatment not included)
• 20% off any other treatment needed
• ** Regular cost for above services: $780+

$299 Child Plan Membership (<14 yrs.) includes:

• Two regular dental cleanings
• One initial comprehensive exam
• One annual exam
• Associated X-rays (full set if necessary) and yearly bitewings
• One emergency exam during a one-year period (no treatment)
• Two fluoride treatments
• 20% off any other treatment needed
• ** Regular cost for above services: $784+

Membership Plan Terms

We offer an In-house Patient Savings Plan designed specifically for patients with NO dental insurance, providing better access to quality care at an affordable price.

• Premiums are yearly; benefits expire one year from the date of enrollment if not renewed
• Must be paid in full on the date of enrollment
• Procedures included in the plan must be used within the premium year
• Unused portions cannot be carried over to the next year
• MEMBERSHIP FEE IS NON-REFUNDABLE
• For additional services, payment is due in full at the time of service unless other financial arrangements are made in advance
• The patient/responsible party must keep track of plan expiration date and scheduled appointments
• The plan is non-transferable to another person
• Membership is valid only at Dr. Bruce Hulscher & Associates
• Cannot be used with any other offers/discounts
• Any cancellations not made within 48 hours prior to the appointment, or any missed appointments may be subject to a penalty including possible cancellation of the membership plan
• Members are not eligible for interest-free third-party financing (CareCredit).
